General terms, conditions and application format for classification of restaurants



     
Supporting Documents

  1. Copy of Land Use Permit from local authorities
  2. Certificate/ licence from Panchayath/ Municipality/ Corporation to show that your establishment is registered as a restaurant.
  3. Clearance Certificate from Municipal Health Officer/ Sanitary Inspector giving clearance to your establishment from sanitary/ hygienic point of view.
  4. Sanctioned building plans/ occupancy certificate.
  5. If classified earlier, a copy of the earlier "Certificate of Classification issued by Department of Tourism".
  6. Copy of Memorandum and Articles of Association (If company)
  7. If partnership: - Copy of Partnership deed and certificate of Registration (attested by notary public)
  8. Lease agreement in case of leased property (Minimum lease period should be more that 3 years)
  9. Possession Certificate from Village Office
  10. Tax receipt (land) from village office
  11. Tax receipt (building) from LSGD
  12. Professional tax receipt from LSGD
  13. Approved Plan
  14. Building permit from LSGD
  15. Food Safety Certificate from Food Safety and Standards Authority of India (FSSAI)
  16. No objection certificate from Police Authority
  17. Clearance from Pollution Control Board
  18. Clearance from Forest and Environment Authority
  19. Clearance from CRZ Authority
  20. Clearance certificate from Sanitary Officer
  21. Fire and Safety Certificate or certificate of installing fire fighting equipments
  22. Certificate of staff trained in fire fighting drill
  23. Staff details (Name, Designation, Qualification)
  24. Staff Qualification certificate (10% for 1 star, 15% for 2 star, 20% for 3 star. 40% for 4 star, 50% for 5 star)
  25. First aid training certificate of staff in the unit
  26. Fitness certificate of staff
  27. If proprietary: - Certificate of Registration (attested by notary public)
